Most common cause
CREATE (or RENAME) of a table, view, index or alias with a name already used in the schema.
Other causes
- An install script run again.
- A constraint with a name already in use.
How to fix it
Use CREATE OR REPLACE where allowed, or check first in QSYS2.SYSTABLES.
